A study published Tuesday in the *British Medical Journal* found chronic traumatic encephalopathy in 25% of former NFL players who died between 2016 and 2021. The figure—142 of 571 deceased former players examined—marks the first population-level estimate not reliant on brain donations skewed toward symptomatic donors. The NFL's previous line, that CTE prevalence remains unknown because families of affected players self-select into donation programs, now carries less weight.

The researchers used insurance claims, death certificates, and next-of-kin interviews to build the sample. 75% of the 571 families contacted agreed to post-mortem neuropathology. The study controlled for career length, position, and documented concussion history. Linemen and linebackers showed 31% prevalence; skill-position players came in at 21%. Players with careers longer than eight seasons hit 34%. The league has not yet commented on methodology, but three people familiar with internal discussions say the union's medical advisory board received advance notice and found no fatal flaws.

This puts actuarial pressure on the NFL's $1.4 billion concussion settlement fund, which has already paid $1.1 billion to roughly 1,600 former players through early 2024. The settlement assumed CTE prevalence below 15% in the general retiree pool. At 25%, the fund's tail risk expands. Two reinsurers who backstop portions of the league's long-term disability coverage have quietly requested renegotiation talks, according to a broker who works both sides. If prevalence climbs toward 30% in future studies, the fund's remaining $300 million becomes a three-year runway, not a decade.

Helmet manufacturers are the second-order target. Riddell and Schutt both face ongoing product-liability cases in Louisiana and Pennsylvania. Plaintiffs' attorneys in those cases will now cite the BMJ study in every summary-judgment brief. The 25% figure also pressures the league's annual $250 million spend on health and safety research, much of it funneled through the NFL Players Association's joint committee. Owners have resisted expanding that budget, arguing most gains come from rule changes, not equipment. This study suggests the rule changes—targeting penalties, kickoff redesigns, limited contact in practice—have not bent the curve enough.

The union's executive committee meets in two weeks. Three agenda items worth tracking: whether to reopen disability-benefit formulas in the next CBA negotiation (current deal runs through 2030), whether to push for independent neurological monitoring at all 32 facilities, and whether to demand equity stakes in helmet companies as a condition of continued product endorsement. The league's broadcast partners have also taken note. One network executive said his team is "stress-testing the value of retired-player content" in light of potential public-perception risk. Translation: fewer legends in the booth if the legends become litigation exhibits.

The study's lead author told reporters the 25% figure is "almost certainly conservative" because families of players who died from overdoses, car accidents, or other non-neurological causes were less likely to consent to autopsy. If real prevalence runs closer to 30% or 35%, the league's entire post-career health apparatus—insurance, pensions, media deals that rely on alumni—needs repricing. The actuaries are already working nights.
